an online method to report Harassment/Bullying

This form is used to report incidents of harassment-racial/sexual that may be of a physical,
verbal or emotional and social form.  We understand that sometimes identities sometimes
need to be protected.  This form will be sent directly to the Race Relations/Cross
Cultural Understanding and Human Rights Advisor (RCH Advisor) of YCMHS. 
***You must understand that without any contact information on your end... it may be
difficult to pursue due to the fact that we need proof of incidents.

*Only report issues that relate to racism/bullying/harassment.  IP addresses can be used
to track submissions that break this rule.
